Insurance: We are in-network for Blue Cross Blue Shield and most Cigna plans and can file these for you. We can check the coverage for you if you provide s with your ID number date of birth and address and phone. If you have out-of state BCBS or Cigna plans, we can also check coverage. IWe can also file with ohter insurers, but request that you pay in advance or provide a valid credit card until we can determine coverage.
We can also provide you with a signed receipt or completed insurance claim form for your payment, which may file with your insurance company.
It is your responsibility to determine what your insurance will pay (prior to any scheduled treatment.)
Phone and e-mail follow-up sessions can be purchased in advance in hourly increments for $150.00 and used in 20-minute increments
-Test of Caloric Needs (Metabolism Testing): $50.00 (must pay for appointment in addition to testing) - Grocery Store/Home/Restaurant Sessions: $200.00 - Menu Planning/Cooking/Grocery Shopping Services - individually priced

Payment for appointments is requested in advance of the scheduled appointment-as a deposit to reserve your appointment-by cash, check, Master Card, Visa, or by PayPal at the bottom of the Client Registration page
